Roger looked at me with a look of complete confusion, "Dan, what's going on, what's wrong, do you know this boy?" he asked. "Yes" I sobbed in reply feeling my normal calm and detached persona running away from me, quickly heading towards hysteria. "His name is Dylan Henderson, he's 16 years old and I know him very well as he's my boyfriend." I felt the whole room look at me. "Ok, well not to sound harsh Dan but we haven't got time for this I'm sorry, we have a patient to treat and its clear you're unable to continue" said Roger trying to take control of the situation. "No way Roger" I said trying to get hold of my emotions, "I am not leaving Dylan now, he needs me more than ever so I'm here and I'm staying." I knew that Roger was already shorthanded and without me Dylan's chances were far worse with me leaving than with me staying. "Ok well we don't have time to argue, let's get to work people, we have a patient to save and it effects one of our own, so we know what we need to do" Roger instructed as we all got to work. I tried to tune out my emotions but I couldn't, my boyfriend was lying in front of me fighting for his life and I was struggling to focus. Before I continued what I had started earlier I leant down to Dylan's ear, "I am here baby, it's me, your Doc, I am going to fix you baby so you stay with me you hear. I love you so much and I can't be without you so you need to pull through, you have to pull through". I pulled away, tears streaming down my face again as I continued with what I had started before he arrived; putting lines of fluid into Dylan, trying to compensate for any blood loss and to get his already dangerously low blood pressure stabilized as well as taking blood samples to send away for immediate analysis. For the first time since I had started in the ER I noticed the noise of all the machines, the slow and labored "beep, beep, beep" of the heart rate monitor telling us that Dylan's heart was struggling. The clunking and clicking of scissors as the nurses cut off his clothes to give everyone access to the body of my beautiful love. Roger tubed Dylan, opening his airway and connecting him to the ventilator. "Right team let's get the X-rays done please, let's see what going on inside" Roger said. The team responded moving our portable X-ray into position to take a full set of X-rays. We started the X-rays when the heart rate monitor started to chime. "Oh shit he's going to VF everybody" Roger exclaimed as the senior nurse dragged to crash trolley over to the side of the bed. "Move the x-ray out the way please." The chiming of the monitor is a doctor's worst nightmare; it means the heart isn't functioning properly and VF (Ventricular Fibrillation) if not remedied within around 90 seconds usually leads to the death of the patient. My blood went cold and I felt faint at the thought of my gorgeous boyfriend dying in front of me on the table. "Come on Dylan" I whispered quietly "Come on." "Charge to 200 please" instructed Roger, "Charging, stand clear please, shocking." Dylan's body jumped in response to the electric shock and I felt physically sick. "No change, charge to 250 please, charging, stand clear, shocking." I stood watching in silence praying that I wasn't watching the last moments of my boyfriend's life. "No change, charge to 300 please, charging, stand clear, shocking." Dylan's body jumped again and the machine stopped chiming and reverted to its normal "beep, beep, beep". "Good, ok people he's back with us, we have normal sinus rhythm let's get back to the x-rays" said Roger. All the time that Dylan had been in VF it had been like I was watching in black and white and slow motion. For the next 15 minutes the team worked tirelessly finishing off the initial emergency treatments as well as completing all of Dylan's x-rays. When the x-rays were ready I joined Roger over at the screen to review the films. "Ok Dan let's see what we have here" Roger said. "Holy fuck" I exclaimed as we looked at Dylan's films. "Doesn't look pretty does it?" Roger said looking at me. "He has pneumothorax on the right, fractured clavicle on the right, multiple fractures to the right Tib and Fib and this dark patch here indicates that he's possibly got some internal bleeding that we'll need to scan to see further. However on the positive side his pelvis is intact his spine is completely fine, his head injury, initially at least, looks fairly superficial but we'll have to closely monitor to ensure there is no post trauma swelling but his pupil response is normal so fingers crossed." Roger looked at me and I knew what he was going to say. "Dan I don't need to tell you how seriously injured Dylan is, you know these injuries are definitely survivable, however the next 24 hours are critical, his youth definitely helps him but he's got a long way to go before we can say he's going to be out of the woods." I knew what Roger told me was true as tears started to gently and silently roll down my cheeks as I faced up to the possibility that Dylan might not make it. "Thanks Roger I appreciate everything you're doing" I said in a barely audible whisper. "Come on let's go and get his breathing stabilized by sorting out his pneumothorax." We moved back over the Dylan as Roger and I carried out the short but fairly invasive surgical procedure to re-inflate Dylan's lung. With that done the surgical team came down to review Dylan's case and understand what they would need to do and how quickly. When the surgeons had finished their assessment they announced that they would be taking Dylan up immediately to work on his fractures and explore for the possibility of abdominal internal. I walked over to Dylan and kissed him on the cheek and forehead, "See you when you're out of surgery baby, I love you, please be strong" I whispered in his ear knowing I would get no reply. As I moved away the porters grabbed hold of the gurney and wheeled Dylan from the trauma room.
